What Did the Study Find?
A recent international, multicentre randomized controlled trial published in the New England Journal of Medicinecompared the combination of finerenone (a mineralocorticoid receptor antagonist) and empagliflozin (an SGLT2 inhibitor) versus each medication alone.
The study enrolled 800 patients with CKD and type 2 diabetes who were already on standard therapy with renin–angiotensin system (RAS) blockers. Participants were randomly assigned to receive:
- Finerenone + placebo
- Empagliflozin + placebo
- Combination of finerenone and empagliflozin
The primary outcome was the change in urinary albumin-to-creatinine ratio (UACR)—a key marker of kidney damage—from baseline to 180 days.
Key Results
- Combination therapy reduced UACR by 29% more than finerenone alone, and 32% more than empagliflozin alone.
- The majority of UACR reduction occurred within 4 weeks of starting therapy.
- No major safety concerns were identified, with low rates of side effects such as hypotension, hyperkalemia, and acute kidney injury.
Bottom line: Starting both medications together may protect your kidneys faster and more effectively than either drug on its own.
