Progestin-Only Birth Control May Increase Risk of Asthma Attacks in Women Under 35

What Is Progestin-Only Birth Control?


Progestin-only oral contraceptives (often called the “mini-pill”) are a popular choice for birth control—especially for women who cannot take estrogen. But a new study published in ERJ Open Research has raised concerns for women with asthma, especially those under 35.


The research, which followed over 130,000 women aged 18 to 50 with asthma, found a striking trend: those under 35 using progestin-only pills had a 39% greater risk of asthma attacks compared to women who were not on any hormonal birth control.

Why Is This Happening?


The study suggests that excess progesterone—from both the body (endogenous) and birth control (exogenous)—may be to blame. Progesterone is known to affect airway inflammation and may increase bronchial sensitivity, leading to more frequent or severe asthma symptoms.


Notably, this effect was not observed in women over 35, as natural progesterone levels begin to decline with age.

Key Findings from the Study:

  • 39% increased risk of asthma attacks in women under 35 using progestin-only pills
  • 24% increased risk in women with high eosinophil levels (a type of white blood cell linked to inflammation)
  • 20% increased risk when low corticosteroid use is combined with progestin-only pills
  • No increased risk with combined oral contraceptives (estrogen + progestin), suggesting progesterone alone may drive the risk
